This online form can accommodate up to 5 family members/lesson slots. Please take a moment to read TMC's POLICIES as you will be asked to electronically sign and date the form below agreeing to them.

NOTE: A student wanting to learn 2 or more instruments will require filling in Lesson Slot 1 and Lesson Slot 2 (or more). Certain Fields are required for submission. Please make sure to include AVAILABILITY.
